Lisa Zouvelos Kreag, MSW, LCSW

Executive Director

Lisa has been the Executive Director since August 2019. Lisa graduated from Indiana University in 1997 with her Master of Social Work degree. She is a licensed clinical social worker and has been practicing in hospitals, child welfare, health coaching and tele-health for over 28 years. She is a lifelong resident of Hamilton County and has lived the last 26 years in Westfield. She has been involved in the community through volunteering with girl scouts, sports teams, organizing and various board positions. She began her relationship with Heart and Soul Clinic as a volunteer. She is passionate about helping reduce barriers for individuals that prevent good health.

Barbara D. Haehner, MD - Board Certified Adult Internist and Nephrologist, Pharmacist

Dr. Haehner graduated from Purdue University with a Bachelor of Science in Pharmacy in 1985. She graduated from the Indiana University School of Medicine residency in 1989 and subsequently completed an Internal Medicine residency in 1992 and then went on to complete a combined fellowship in Nephrology and Clinical Pharmacology in 1994. She served as an Assistant Professor at the Indiana University School of Medicine through 1999 where she did Pharmacology bench and clinical research, as well as clinical Nephrology.

Dr. Haehner joined a private Nephrology practice in 1999. (Nephrology and Internal Medicine-now Indiana Kidney Specialists), served in the Indiana University Health system as well as the Franciscan Health system, as a clinical Nephrologist. She also served as the medical director of the DSI Greenwood dialysis unit. She served in this capacity until her recent retirement in June of 2015.

During all of her adult life, Dr. Haehner has lived and served in the central Indiana area. She currently attends and is very active at Lifepointe Church, which recently relocated from Carmel to the Westfield area. She has served on the board of directors at the church since 2006. Serving others in the Name of Christ is very important to her.

Since her recent retirement from private practice, Dr. Haehner has felt led to take on the service of becoming the Medical Director of the Heart and Soul Free Clinic (as well as serving on the board). Her hope is to help increase the scope of medical and dental services we have to offer to the uninsured and underinsured residents of Westfield and surrounding communities.

Tayon Swafford, MSW, LSW

Mental Health Director

Tayon R. Swafford (He/Him/His) is a PhD Candidate at the Indiana University School of Social Work (IUSSW) and an adjunct instructor of Social Work at Indiana University and the University of Tennessee. At these universities, Tayon offers undergraduate and graduate courses on Diversity and Social Justice, Social Policy and Programs, Spirituality and Clinical Social Work, Evidence-Based Practice for Group and Individual Adults, Research Methods, and Statistical Reasoning. Within the research arena, Tayon is working on his dissertation, which explores how Black gay fathers negotiate race, sexuality, and parenthood. When he is not wearing his social work hat, Tayon offers undergraduate courses in religious studies at the University of Indianapolis.

Tayon’s love for teaching and research is paralleled by his love for community service. In addition to proudly serving as Heart and Soul’s inaugural Mental Health Director, Tayon serves as a board member for Trinity Haven in Indianapolis, Indiana. Prior to commencing his educational journey in the PhD program at IUSSW, Tayon worked as a Family Case Manager and Trainer for the Indiana Department of Child Services. Tayon holds a Master of Social Work (MSW) degree and a Graduate Certificate in e-Social Work Practice (GCESWP) from IUSSW. Tayon also holds a Master of Theological Studies (MTS) degree from Vanderbilt University and a Bachelor of Arts (BA) degree from Albright College in Religious Studies.

 

LuAnn Pronschinske

LuAnn joined the staff of the Heart and Soul Clinic in January 2016. She has a BS degree in Accounting with a minor in Computer Science. LuAnn spent several years as a Staff Accountant before devoting her time to staying home and raising her 3 children. She has been very active in volunteering in the schools, at her church and in the community. She is especially passionate about advocating for children and adults with disabilities. She loves the opportunity to serve those in need in the local community as well as work with an amazing, dedicated group of volunteers.

Tami joined the staff in January 2022 after working in physical therapy for 27 years. Serving and caring for others has always been at the core of her profession. She is excited to work alongside so many dedicated medical staff and volunteers to provide exceptional care to our patients.

Jennifer Elgin MSN, APRN, FNP-C

Nurse Practitioner

Jennifer started as an RN volunteer at the Heart and Soul clinic in 2021. Jennifer graduated from Indiana University School of Nursing in 1994 and received her Family Nurse Practitioner degree from Indiana Wesleyan University. She has over 30 years of diverse adult and pediatric clinical experience in various settings including acute care, outpatient, and home care.

Jennifer grew up in southern Indiana. She has been happily married for 30 years and has two grown children. Faith, family, and serving others is her passion and she has been very active in volunteering in her community.

During her adult life she has lived and served patients in Indiana, Michigan, and California. Jennifer is committed to delivering high quality, evidenced based, compassionate care, and fostering a long-term trusted relationship with patients and families. She is a member of Sigma Theta Tau, The Coalition of Advanced Practice Registered Nurses of Indiana, and The American Association of Nurse Practitioners.

Linda Jackson, RN

Diabetes Educator

Linda, a native New Yorker, received her BS degree in nursing from Columbia University in 1974 and worked full time in several different hospital settings for 6 years. She then became a stay at home mom for several years. In 1991 she took a part-time job as a pediatric home care nurse; a position she held for almost 30 years. Ten years ago she moved to Westfield and became a nurse volunteer at the Heart and Soul Clinic. In 2018 she attended the annual conference of the American College of Lifestyle Medicine in Indianapolis, where she met her “tribe”: a group of medical professionals who desire to promote positive lifestyle change as essential for best health. After becoming a certified lifestyle medicine practitioner, she completed an 18 hour course on Type 2 Diabetes Remission. Knowing that diabetes affects many clients at the clinic, she offered to become Heart and Soul’s first diabetes educator in 2023 (a position she now calls her “best job ever”).

Darci Barr

Risk and Compliance Coordinator

Having began her relationship with Heart and Soul as a volunteer, Darci brings over 20 years’ experience in dentistry, as a Program Chairman of two expanded functions dental assisting programs, her expertise in post-secondary education, program development, and management was emphasized by an award by Indiana Workforce Development. Mrs. Barr has served at a national level as a subject matter expert for the Dental Assisting National Board publications. 

She held the position of Board President for 2 years while serving as a founding board member for the DALE Foundation during her six-year appointment, 2010 -2016. Darci’s career contributions lent authorship works for F.A. Davis, Cengage Publishing, Pearson Education, Dr. Bicuspid, The Dental Advisor, the American Dental Education Association (ADEA), among others. She received her master’s degree in Human Rights (research) from the University of London, School of Advance Studies, following her Bachelor’s in Education from Indiana State University. 

An advocate for humanitarian efforts, Mrs. Barr’s consultancy and volunteerism has expanded from advisory roles in dental organizations into board governance for various non-profit organizations. She is honored to serve the mission of the Heart and Soul Free Clinic as the Dental Clinic Operations Manager.

Molly Clark

Molly joined the staff in November 2024 after volunteering at the clinic since 2015.  She has a passion for serving the community and has a background in health informatics. 

Myrna Ortiz

Office Assistant

Myrna joined the staff at the clinic in September 2021. She is a great asset to the clinic with her bilingual skills. She has years of working in the banking industry and she loves the opportunity to help the community.
